The Behavioral Health Care Manager (BHCM) will assist members with appropriate specialized behavioral health care coordination and care management services to achieve their optimal levels of mental health. The BHCM will report to the BH Supervisor, Integrated Health Care Management (IHCM). The BHCM shall meet the requirements for a LPC or LCSW and have at least two to three (2-3) years experience and expertise in the special behavioral health needs. The BHCM will be responsible for engaging members, member caregivers, and providers to assess, plan and establish individual member goals. The BHCM will facilitate and coordinate care for members while assuring quality and use of cost-effective resources.

Under the supervision of the IHCM Behavioral Health Supervisor, the Rapid Response Behavioral Health Care Manager is responsible for providing select program interventions according to established health management program guidelines and the member population. The BHCM will be responsible for promoting physical and behavioral health integration and outreach activities with members, member's caregivers and providers. The BHCM will serve as a single point of contact dedicated to liaising with the judicial system, PASRR program and CSoC program.


  • Licensed Mendel Health Professional (LMHP) required: LCSW (Licensed Clinical Social Worker) or LPC (Licensed Professional Counselor) Preferred
  • Two to three (2-3) years experience and expertise in the special behavioral health needs.
  • Clinical post graduate degree (LCSW) must be from an institution that is fully accredited by a nationally recognized educational accreditation organization and the individual must have completed a supervised field experience in case management, health or behavioral health as part of the degree requirements.
